Community Aged Care Packages
CACP (Community Aged Care Packages) assists older persons to remain at home and continue to live independent lives. The aim of the program is to provide a comprehensive range of services to people in their homes in order to avoid premature institutionalisation. Co.As.It delivers culturally and linguistically appropriate CACP aimed at older Italians, offering them a service provided by bilingual workers who are also familiar with the Italian culture.
CACP is targeted to the ageing population living at home who have:
- A variety of care needs such as physical, medical, social and emotional
- A need for services that are not provided/available from other community services
- A preference to remain home with appropriate support
- A need for ongoing monitoring and review of changing care needs
- Access difficulties to other services.
The range of services available to clients under this program include:
- Showering/bathing or personal hygiene
- Assistance with dressing
- Light housework
- Laundry assistance
- Home respite for carers
- Shopping
- Assistance with meal preparation
- Socialisation
- Transport to and from appointments, eg medical
- Culturally relevant assistance eg. Social outings or church
Fee:
This will be negotiated with the recipient according to the
services provided, and as outlined by the funding body.
Target Group:
The ageing members of the Italian community who require assistance to
continue to live independently in their own homes.
