Co.As.It. Home Care offers services that are sensitive to cultural, linguistic and clinical care needs to ensure remain independent in your own home, for longer. We offer a wide range of tailored support services that can provide as little or as much help you need to enjoy your life at home.

Our services are available throughout Sydney. The services available can vary based on where you live and whether you are accessing government funding. Whether you receive funding from the Commonwealth Home Support Program (CHSP), Support at Home (SAH), or are self-funded, our Care Team can support your access to high quality services at the best available value.

Using your Support at Home funds

We can help you with:

In Home Nursing

We can provide qualified nurses to care for your health at home, from managing ongoing illness to medications, and pain management to wound care.

Allied Health Services

We can provide experienced healthcare professionals to support your physical and mental well-being at home, from physiotherapy to podiatry, and dietetics to massage therapy.

Case Management and Support

We can provide expert support and advice to help you design services based on your preferences and aspirations.

Personal Care

We can provide dignified personal care at home, from showering to grooming, and continence management to assistance at mealtimes.

Social Support

We can support you to stay connected and engage with your local community, from attending a group outing to starting a new hobby, and seeing a movie to joining an exercise classes.

Respite Care

We can provide a range of respite care options, to provide you and your regular carer with a well-deserved break, from help with meals to transport, and social support to transition from hospital to home.

Goods and Equipment

We can provide goods and equipment you need to ensure you remain feeling confident and safe in your home and community, from mobility aids to wheelchairs, and toileting supports to therapy aids.

Home Modifications

We can assist you to make some minor alterations in and around your home, from installation of rails and ramps to small upgrades to bathrooms.

Transport

We can arrange safe and comfortable transport, from attending an appointment to going shopping, and seeing a movie to visiting family or friends.

Domestic Assistance

We can help with all those jobs in and around the home, from vacuuming to dusting, and washing the dishes to doing the laundry.

Home Maintenance and Gardening

Our experienced and reliable team can carry out small maintenance jobs in and around the home and garden, from lawn mowing to gardening, and changing light bulbs to simple plumbing.

Meals and Grocery Shopping

We can provide support with the grocery shopping, cooking or having ready-made meals delivered to your home.

Support at Home Services:

Under the new Support at Home guidelines, services have now been divided into three categories to align with new contribution requirements.

  1. Essential Clinical
    Specialised services to maintain or regain functional and/or cognitive capabilities.
  2. Independence
    Support delivered to older people to help them manage activities of daily living and the loss of skills required to live independently.
  3. Everyday Living
    Support to assist older people to keep their home in a livable state in order to enable them to stay independent in their homes.

Essential Clinical services will not require any contributions from clients. Independence and Everyday Living services will now require Transitional/Hybrid and new Support at Home recipients to make a contribution to their service. These contributions are calculated based on your income assessment.
